Parental Consent Form for Off-Site Youth Activities (Non-Residential)

 

If you're here, then an event is coming up, and you need your parent's to ok it for you...

Well, here's how you do it!

 

1) Print this out:  Parental Consent Form (pdf) 

2) Get your parent/guardian to fill ALL of it out and then sign it.

3) Bring it and the fee (if any) for the trip back to church by the date mentioned in the publicity for the event.

 

It's just that easy!

 

Please note, this is NOT the correct form for an OVERNIGHT/RESIDENTIAL trip. Those forms should be made available a few weeks before the booking deadline.
